Slack Notification: add Monitor Name#908

The slack notification is unable to be changed between monitors. Which means my current notification is just “something is down, I don’t know what”; not terribly useful given the number of monitors. But it should be relatively simple for a dev to concat the monitor name with the entered notification before sending it to the API. e.g. (assuming the Monitor’s name is Main Page) Main Page: something is down, I don’t know what
