Alert storm protection / grouped notifications#1600

When many monitors go down at once, getting one alert per monitor can create too much noise and make incidents harder to manage.

It would be useful to detect these situations automatically and switch from individual alerts to one grouped storm alert, then send a single recovery alert once everything is back up.

This would be configurable, for example by threshold, time window, and monitor group, so unrelated incidents stay separate.
